In order for us to open a file, please follow these steps:
- Download the welcome form at the following link: click here ; To do this, click on File then Download then Microsoft Word or PDF Document.
- Each person involved in the process completes the form: for relational therapy, you should have more than one form. If certain questions make you uncomfortable, feel free to skip them.
- Send the form(s) by email to the following address: info@cliniquemestra.com
You will receive an email confirming your registration to the waiting list. Please note that due to high demand, response times may be lengthy.
